Membership Conditions
1. Every July, membership prices may be subject to a CPI rise.
2. Members must ensure that the appropriate dress standards are maintained. A sweat towel must be used at all times during
workouts, closed shoes must be worn at all times.
3. Members must register at the 24-hour card reader or the front reception reader upon arrival and scan their membership fob tag.
Providing access to the facility to non-members or unaccompanied 14-17-year-olds without 24/7 access, will result in a $250.00 fine
each, a 1 Week access suspension and may lead to cancellation of membership
4. No food or drink, except water and sport drinks, are allowed within the facility.
5. Smoking is not permitted at all within (including car park and entry way) of facility.
7. No private business, including personal training, may be conducted within Life and Soul Mayfair without the prior written consent of
Management.
8. A parent/guardian must supervise persons under the age of 16, unless authorized by Management.
9. Members must notify Life and Soul Mayfair in writing of any change of address.
10. Members can suspend their membership on Direct Debit memberships only (Excludes Short Term or Promotional Membership
Periods). A direct debit membership has unlimited suspension** through the term of the membership – for a Minimum Period of 7 days
and a Maximum of 6mths**. Direct debit minimum term payment instalment will be suspended during the period of suspension and will
resume once the period of suspension finishes. An Online suspension request form is required to be completed. All Suspension
Request applications must be submitted a minimum of 48 hours prior to commencement of suspension period for Club Administration
to process application before the Friday weekly debits. Life and Soul Health Clubs cannot backdate suspension applications. You must
submit a separate Suspension Request application online for any extensions on current holds or normal pay schedule will recommence
once the on-hold period has expired
11. Management reserves the right to refuse or cancel the membership of any person.
12. Management reserves the right to review and implement new pricing structures at the beginning of each month as they see fit.
13. Should any question or interpretation on construction of these rules and regulations arise, then the decision by Management in
relation to that question shall be final and no claim for compensation, damages or refund may be made by the members against Life
and Soul Mayfair. Any claims for compensation, damages or refunds against Life and Soul Mayfair are subject to the approval by
Management.
14. Members will not be reimbursed for lost days during their membership due to public holidays, Christmas breaks or the like, or due
to the closure of the club due to unforeseen circumstances.
15. Completion of the membership agreement and receipt of first payment is taken on confirmation of membership.
16. Once outside the minimum term of your membership, changes to your membership without penalty are possible, provided you give
at least 28 days’ notice in writing.
17. A cooling off period of 7 Days applies to all new membership contracts, effective from time and date of signing of agreement (less a
$75 administration fee).
18. Membership is fixed for the term of the arrangement.
19. Members must agree to follow all Life and Soul Mayfair terms and conditions.
20. I agree that the membership fees are not refundable, and I agree and understand that non usage of the facility does not allow early
termination of this agreement, nor does it warrant a refund of any type. I also agree and understand that non usage of the facility does
not affect any payment terms and conditions of this agreement, and that Life and Soul Mayfair is in no way responsible for my level of
usage of the facility.

Minimum Term Contract & No lock in contract

Cooling Off Period 
1. This membership is subject to a 7 Day Cooling off period. This means you can cancel your membership agreement, without needing
to give cause or reason, before the end of 7 Days immediately after both you and Life and Soul Mayfair representative have signed the
membership agreement. This cancellation must be in writing and given to us before the end of the 7 Day cooling off period, in order to
be effective. A refund will be given, less administration fee of $75 and any costs incurred during the membership i.e. fitness services,
products etc. No refunds will be given after the 7 Day Cooling off period

Payment Declaration
I agree that the liability for the direct debit shall continue as agreed, notwithstanding my frequency of attendance at the studio or should
I cease to attend altogether. I shall not be entitled to any refunds for any reason. As a member I shall cease to have any rights to the
centre should full payment not be received by the due date, at the discretion of Management.
As a member, I shall pay all costs of the collection of the monies due including any debt collector fees, legal fees or other costs payable
by my defaults.
I acknowledge that I have read and understood all of the above provisions as well as the Membership Conditions and acknowledge that
my membership is a legally binding contract and I agree to observe and be bound by it.
Direct Debit Conditions
1. All conditions stated in Ezidebit Billing Form shall apply. Weekly debits are deducted every Friday, and Initial debit on day of sign up
2. Early Cancellation fee $199.00 will be charged if membership is cancelled inside the selected minimum term
3. Cancellation Request Form to be completed and returned with 28 days’ prior written notice for all cancellations. The onus is on the
payer to confirm receipt of form.
4. All contracts are based on a minimum 3mths and automatically renew, payments will continue until further notice or until written
notice is provided to cancel the membership contract. Written notice is to be provided 28 days prior to the renewal date for cancellation
to be processed at the end of an 3mth minimum contract. All payments within the 3mth minimum are required to be paid.
5. Initial debit is deducted on the day of sign up – all future debits will be deducted every Friday

Photography & Media Release
1. By entering and using the facilities at Life & Soul Health Clubs you consent to being photographed or recorded for promotional, social
media, and marketing purposes. These images or videos may be used in digital or print materials, including but not limited to our
website, social media pages, and promotional content.
2. If you prefer not to be included in any photography or media, please inform a staff member in writing or email us at
admin@lifeandsoul.com.au
3. We respect your privacy and will always handle media usage with care and discretion.
